The primary purpose of this study is to use the "Get With the Guidelines" program (GG) to analyze heart failure measurements and the quality of patient care at the University of California Irvine Medical Center (UCI). Additionally, this study will observe the effectiveness of the quality improvement technique, Plan-Do-Study-Act (PDSA), in assisting the center to improve the lowest score item monitored by the GG program, the discharge instruction sheets. This study found that the discharge instruction sheets were not completely filled out by the residence and the attendee. Important information was missing and patients lacked information on the discharge sheets about medication instructions in the event that patient's symptoms get worse.
