Congestive heart failure (CHF) patients have a higher risk of hospital readmission due to complications associated with the disease. The CDC indicates that CHF affects about 5.7 million Americans; thus, there is a need to identify resources to protect patients and prevent future readmission rates. The current project was aimed at determining the impact of an educational intervention on CHF patients, specifically reducing readmission rates. The participants included a convenience sample of adult patients aged up to 65 years who had been previously hospitalized for CHF and who have been prescribed heart failure medications. This group of patients provided greater insight regarding the importance of education to inform patients of self-care and other actions that may be taken to prevent complications associated with CHF. Data analysis was conducted using decretive and independent samples t-test at 0.05 significance level. The results indicated a statistically significant decrease in the between the pre-intervention (M = 2.85) and post-intervention (M = .20), mean length of stay in days, t (38) = 12.261, p < 0.001. These findings can contribute to positive social change by promoting CHF awareness and minimizing healthcare costs.
