Case Management Program for Dementia Caregivers: A Grant Proposal

Abstract (Summary)

The purpose of this project was to write a grant proposal to fund a program to address the needs of caregivers of family members with dementia. Independence at Home (IAH), a non-profit community-based program offered through the Senior Care Action Network (SCAN), was selected as the host agency for this program. Based on a review of literature, the grant writer was able to identify the interventions most needed by dementia caregivers. The grant writer designed a program called the Supportive Services Caregivers Program (SSCP), which includes: case management, caregiver trainings, support groups, homemaker assistance, transportation, and linkage to community resources. Through providing these services, the goal would be to reduce burden and improve the well-being of caregivers. The grant writer selected the California Community Foundation (CCF) as the funder of this project. The submission or funding for this grant was not a fulfillment for the successful completion of the project.
