The purpose of this project was to write a grant proposal and identify potential funding sources to implement a program to provide in-home transitional care services to older adults in south Orange County. The transitional care program targets older adults who are at risk for hospital readmission and aims to increase their quality of life. A considerable amount of research was conducted on existing literature regarding older adults, the population increase, and the risk factors they face. The Archstone Foundation was selected as the best potential funding source for this project after researching via the Internet and grant databases. A grant proposal was then written to fund a program at Age Well Senior Services in Laguna Woods, California. The actual funding and/or submission of this grant were not a requirement for the successful completion of this project.
