An outreach program for homeless individuals: A grant proposal

Abstract (Summary)

The purpose of this project was to write a grant to design a program called the Outreach Program for the Committee on the Shelterless (COTS) that would address the needs of homeless individuals in Sonoma County. An extensive literature review about outreach programs, national and local homelessness was conducted to pinpoint service needs. The Community Foundation of Sonoma County's Health and Human Services grant was chosen as a potential funder.

The goal of the proposed program is to assess the needs of homeless individuals and to provide individuals with resources and services that will ultimately move them towards permanent housing.

By bringing services offered at COTS to homeless individuals outside the shelter, the program seeks to increase awareness of the shelter, and increase client involvement in activities that will build valuable skill sets. The actual submission and/or funding of this grant were not required for successful completion of this program.
