Abstract (Summary)

The god of this project was to emphasize the importance of providing education to case managers in the home health setting. It is a worthwhile investment by managers in a home health setting to provide the tools and resources that will help case managers to navigate the care management process and take on the various challenges and barriers to provide good quality outcomes. The ultimate goal for effective case management is to improve a patient's well-being and ensure a high level of care. The role of the case manager is crucial to the patient care team and cannot be understated. Effective and continual training, then, is pivotal to ensuring that home health case managers are able to perform their roles of coordinating a patient's care and collaborating among providers to ensure the best health outcomes possible for patients.
